Hospice House & Support Care of Williamsburg (HHSCW) partners with all medical hospice agencies in the greater Williamsburg area to provide 24/7 end-of-life and respite care to patients and families in need of these services at our Hospice House.
All medical professionals seeking to refer a patient and/or family to Hospice House MUST complete a Hospice House Referral Information Form and submit this form to the Hospice House Clinical Director, Heidi Crocker R.N. The Clinical Director will review the patient information to determine eligibility for admission.

Admission Criteria for Services at Hospice House
Hospice House & Support Care of Williamsburg requires that the following criteria be met in order for a patient to be admitted to Hospice House:
- The patient and caregiver are being served by a medical hospice agency.
- The patient’s behavior is not disruptive and does not present a danger to self or others.
- The patient and caregiver agree to a plan for care that includes a projected length of stay and a plan for discharge.
What Care at Hospice House Encompasses
Hospice House is operated by Hospice House & Support Care of Williamsburg, a 501(c)(3) community support social model hospice which provides 24-hour care for terminally ill individuals and their loved ones when dying at home becomes unmanageable. The Hospice House is a spacious residential center designed as a “home away from home.” With four guestrooms, a kitchen, family room and sunroom, surrounded by gardens and woods, Hospice House accommodates up to four guests, with ample space for family and friends to gather.
Families are welcome to remain with the guest as much as they like, but are also encouraged to take ‘time out’ as needed in our shared family quarters that include kitchen and living areas.
There are no sleeping facilities in the house, but a recliner that converts to a twin bed, is available for use in the guest room so that one family member may stay overnight if desirable.. Hospice House is staffed around the clock with team members that are specially trained in hospice and end of life care. Level-of-care at Hospice House is non-medical and supportive. At Hospice House, as in the patient’s own home, medical care continues to be coordinated by the Hospice nurse and personal physician. Although end-of-life care is our priority, we do offer limited respite care if available.
